SUMMARY:

Therapeutic hypothermia has been implemented in Neonatal Intensive Care Units (NICUs) over the past 20 years as a strategy to protect the central nervous system of newborns diagnosed with moderate to severe hypoxic-ischemic encephalopathy. The treatment consists of cooling the head region or the entire body of the newborn, maintaining a temperature between 33.5°C and 34.5°C starting within the first six hours of life and sustaining it for 72 hours until the rewarming phase. Proper implementation requires practical tools to guide and enhance neonatal care. This study aims to validate a therapeutic hypothermia protocol to guide the actions of the multidisciplinary team in treating newborns with perinatal hypoxia admitted to the NICU. It is a methodological study that involved the formation of a remote focus group for discussion and validation of a clinical protocol. The Content Validity Index (CVI) was used by the participants to assess the protocol items. The research was conducted in a NICU of a University Hospital located in the interior of Rio Grande do Norte, which has five neonatal beds. The study population included 48 healthcare professionals, including physicians, nurses, physiotherapists, and nursing technicians. Among them, 29 were selected according to the criteria established by Ministerial Ordinance No. 930 of May 10, 2012, which regulates the functioning and composition of multidisciplinary teams in NICUs. After applying a questionnaire to assess the sample profile and professional qualifications, 12 specialists were chosen to compose the study sample. The participants had between two and ten years of experience in the field, with training at the specialization, master's, and doctoral levels. Most reported partial proficiency in low-temperature practices and expressed insecurity in applying the therapy, highlighting the need for tools that assist the multidisciplinary team and ensure safe treatment implementation. The remote meeting for protocol validation took place in February 2025 via Google Meet, with the participation of specialists, collaborators, and the researcher. Modifications were suggested for some protocol items to improve clarity and the execution of procedures. The content analysis conducted by the judges using the CVI showed scores above 0.78, considered satisfactory according to the evaluation instrument. The suggestions from the focus group were compiled and incorporated into the therapeutic hypothermia protocol, along with the development of a flowchart to optimize the actions of the healthcare team. In parallel, a systematic literature review on the benefits of therapeutic hypothermia in neonatal neuroprotection is underway, serving as a theoretical basis for the research.
