SUMMARY:

The thesis investigates the significant public health challenge posed by congenital syphilis, an infection transmitted from mother to fetus. Syphilis, caused by the bacterium Treponema pallidum, remains a critical issue due to its potential to cause severe consequences, including congenital infection, miscarriage, and stillbirth. This research focuses on the inadequacies in case reporting and management in Brazil, revealing that a substantial proportion of congenital syphilis cases are recorded based on subjective criteria, leading to incorrect notifications and data inaccuracies. Using data from the Notifiable Diseases Information System (SINAN) of the Unified Health System (SUS) and a specific investigation protocol applied in the state of Rio Grande do Norte, the study highlights the need for improved epidemiological surveillance practices and case management to control the vertical transmission of syphilis. The methodology adopted includes a quantitative analysis of data from 2015 to 2020, revealing that 28.70% of the notified cases contained inadequate information, compromising the reliability of these notifications. The thesis proposes a revision of the Clinical Protocol and Therapeutic Guidelines (PCDT) and advocates for changes in SUS's financing logic, emphasizing the importance of meticulous care and investigation of cases to ensure accurate data and a more effective response to the congenital syphilis epidemic in Brazil. The findings underscore the urgent need for integrated efforts between epidemiological surveillance and primary health care to achieve substantial improvements in the prevention and management of congenital syphilis.
