SUMMARY:

Introduction: Amyotrophic Lateral Sclerosis is a progressive
neurodegenerative disease, which presents signs and symptoms resulting
from the degeneration of upper and lower motor neurons. This process leads
to progressive weakness of the appendicular and axial musculature, causing
difficulties in walking and loss of functionality, in addition to other symptoms
such as dysarthria, dysphagia, and dysfunction of respiratory muscles. Since
there is no curative drug treatment for ALS and due to its complex clinical
aspect, this condition requires a multidisciplinary approach to manage the
different clinical manifestations.

Objective: To evaluate the effects of a protocol of home exercises with face-
to-face and remote monitoring on clinical outcomes and treatment adherence
in ALS patients, investigate evidence regarding the performance of muscle
strengthening exercises in this population, and develop health education
content for individuals with ALS and their families.
Methods and results: Two studies were conducted, resulting in three articles
(one published, one submitted, and one for submission), a health education
course available online, and the production of an exercise guide. Study 1
includes the protocol of the randomized and blinded clinical trial that
compares face-to-face and remote monitoring of a home exercise program for
individuals with ALS. Study 2 consists of the protocol of the systematic review
to evaluate the existing evidence regarding peripheral muscle strengthening
exercises in the population with ALS. Study 3 is constituted by the
aforementioned systematic review, which summarizes eight studies
addressing muscle strengthening in individuals with ALS and presents the
impact of this approach on outcomes such as muscle strength, functionality,
and fatigue. The health education course is aimed at caregivers of people
with ALS and covers general information about the disease and essential
aspects for the self-care of these individuals. The exercise guide was
developed according to the main characteristics of each phase of the disease,
encompassing from passive mobilization and stretching exercises to aerobic
and strengthening exercises for upper and lower limbs.
Conclusions: Regarding the findings of the systematic review, motor
interventions for muscle strengthening did not prove to be effective in
individuals with ALS immediately after the intervention. The course developed
and made available online showed good adherence, receiving a maximum
rating in the evaluation by users.
