Introduction: Population aging also affects the increase in the prevalence of chronic and disabling diseases, and provides a paradigm shift, since the health of the elderly is seen from the perspective of the individual's functional capacity and autonomy. Chronic pain in the elderly is a public health problem, as it causes a high demand for health services, being a multifactorial phenomenon, covering physical, emotional, sociocultural and environmental aspects, which can be associated with depression, physical and functional incapacity, isolation change in family dynamics and hopelessness. Objectives: To estimate the prevalence of pain in the sample considering the differences by sex and study location. Identify the behavior of pain in relation to physical performance in the presence of confounding variables. Characterize the sample according to socio-demographic and clinical variables. Methodology: Analytical observational cross-sectional research derived from the multicenter Internartional Mobility in Aging Study (IMIAS), conducted in the cities of Saint-Hyacinthe (Quebec, Canada), Kingston (Ontario Canada), Manizales (Colombia), Tirana (Albania) and in Natal (Brazil). Data from 2000 elderly people evaluated in 2012 were analyzed. For the evaluation, socioeconomic data, health and anthropometric measures were collected. Physical performance was assessed using the Short Physical Performance Battery (SPPB) and the self-report of the presence of pain, back pain and pain in lower limbs (lower limbs), being assessed through a direct question in the interview with each elderly person. The analyzes were performed using the Statistical Package for Social Sciences (SPSS) version 21.0. For all stages, we considered p <0.05 and the 95% confidence intervals. Results: Men had a lower prevalence of pain when compared to women, with the lower limbs region being the most affected, with statistically significant values with p <0.001. For physical performance, men obtained a better total SPPB score, obtaining a score of 9.68 with p <0.001. Feeling pain decreased physical performance in the elderly p <0.001. When analyzing models 1 and 2 of regression, the lower limb pain remained significant for both men and women p <0.001, and it was observed that health variables such as depressive symptoms, body mass index and pain were associated with lower physical performance. in women, with a statistically significant association p <0.001. Socioeconomic conditions such as income, place of residence, showed an association in women for a lower physical performance p <0.001. Conclusion: Pain proved to be a limiting factor, decreasing the physical performance of the elderly population studied and associated factors such as health and socioeconomic conditions were associated with physical performance.
