SUMMARY:

Background: Shoulder pain is related to biomechanical dysfunctions which involve the shoulder complex and possibly the entire kinetic chain.

Objective: To compare the mobility and muscular endurance of the shoulder complex, spine and hip, and the neuromuscular control of the trunk and pelvis of individuals with and without shoulder pain and to verify the discriminative capacity of these variables and relationships.

Study Design: A cross-sectional study.

Methods: A total of 102 individuals were allocated into two groups: with (n=51) and without (n=51) shoulder pain. The range of motion (ROM) of cervical and thoracolumbar spine and hips, the muscular endurance time of cervical and thoracolumbar spine, shoulder and the hips muscles, and the neuromuscular control of the trunk and pelvis were evaluated.

Results: The thesis was divided into two articles, the first of which was entitled: Motor alterations in the kinetic chain in individuals with chronic shoulder pain. The second article, still under construction, is entitled: Relationship between the resistance of muscles of the shoulder complex and the kinetic chain. The results of the first article showed that individuals with shoulder pain presented lower ROM in all movements of the cervical (p<0.01–p=0.04) and thoracolumbar spine (p<0.01–p=0.03) and hip (p=0.01 – p=0.03); shorter endurance time of cervical (p<0.01–p=0.02) and thoracolumbar spine (p<0.01), shoulder (p<0.01) and hip (p<0.01) muscles; and less neuromuscular control of the trunk and pelvis in the compound reach (p<0.01–p=0.01), in the anterior and posteromedial (p<0.01–p=0.04) directions on the ipsilateral and contralateral sides of the shoulder pain. There was a greater perception of pain in individuals with pain during all ROM evaluations (p<0.01–p=0.04) and muscle endurance (p<0.01–p=0.04). Regarding the discriminative capacity, only the endurance time of periscapular muscles of the ipsilateral side to the shoulder pain (AUC=0.80, cut-off point of 56s) and neuromuscular control in the anterior reach of the limb contralateral to the shoulder pain (AUC=0.80, cut-off point of 47.7%) presented excellent capacity to discriminate individuals with shoulder pain.

Conclusion: Individuals with chronic shoulder pain present alterations in mobility, endurance and neuromuscular control of the kinetic chain, with the presence of an increase in painful perception of movement, thus suggesting central sensitization. In addition, the muscular endurance time of the ipsilateral periscapular muscles and the contralateral anterior reach showing excellent discriminative capacity.

Clinical Relevance: Considering the biomechanical alterations in the entire kinetic chain present in individuals with shoulder pain, it is important to include these variables in physiotherapeutic approaches aimed at chronic shoulder pain.
