RESUMO:

Contextualization: Physiotherapeutic activity in patients with Parkinson's disease is mainly aimed at increasing functional capacity and minimizing secondary complications through rehabilitation of movement, in the context of education and support to the individual. Thus, approaches in which the individual can perform therapy, totally or partially, at home, have been proposed. Although the present changes in the trunk region seem to be related to motor and non-motor symptoms of PD individuals, such as postural balance, gait and constipation, few studies have used physical exercises directed to this region to improve some motor symptoms in PD, and no study for improvement of non-motor symptoms.

Objective: To evaluate the effects of a home exercise program on motor and non-motor symptoms of individuals with Parkinson's disease.

Methodology: This is a randomized controlled trial with a sample of 42 individuals diagnosed with idiopathic Parkinson's disease who present symptoms of intestinal constipation (IC), balance and gait deficits, which will be randomized to a control group (CG, n = 21) and experimental group (EG, n = 21). Participants will be evaluated through: clinical-functional evaluation (clinical and anthropometric data); Hoehn & Yahr Disability Scale; Movement Disorders Society - Unified Parkinson's Disease Rating Scale - MDS-UPDRS); level of physical activity (International Physical Activity Questionnaire - IPAQ); proctological evaluation (The Bowel Function in the Community; Bristol Scale); gait analysis using the motion analysis system - Qualisys Motion Capture System®; postural balance analysis, through the force platform; and quality of life assessment (Parkinson Disease Questionnaire-39 - PDQ-39). Both groups will be trained in the Laboratory of Body Practices of UFRN by one of the study therapists, to carry out the protocols at home. The protocol should be performed for three consecutive weeks, in which the CG should perform simple activities: diaphragmatic breathing; stretching of major muscle groups of upper and lower limbs; exercises in the feet; and free upper limb active exercises. While the EG will perform the protocol of exercises focused on the trunk: abdominal strengthening; spine strengthening; pelvic tilt; and contraction of the pelvic floor. Evaluations of outcome measures will occur on the 1st day (initial evaluation), after the end of the interventions (re-evaluation) and 30 days after the end of the interventions (follow-up). The analysis of the data will be performed by a masked evaluator using the Statistical Package for the Social Sciences, through a descriptive analysis of the characterization variables of the sample, through the measures of central tendency and dispersion. The inferential analyzes will consider the statistical significance of 5% for all variables studied. Correlation tests (Pearson or Spearman) will also be applied, after testing the normality of the data. Correlations will be made between the variable constipation and the 'motor' variables: balance and gait. The description of the groups will be presented as mean and standard deviation, and the effect size and the 95% confidence interval will be reported.

Expected results: It is believed that the trunk strengthening program will benefit the participants in the physical scope and can have positive repercussions in the psychological and social scope due to the improvement in the aspects of balance and gait, providing a reduction in the risk of falls. It is also expected that there will be improvement in functional parameters, with an increase in functional capacity. Finally, improvement in the symptoms of constipation, which is a non-motor symptom of the disease, can still be expected. In this way, the expected results attend to the motor and non-motor symptoms; aiming, in this way, to increase the quality of life as a reflection of the improvement of the aforementioned parameters.
