RESUMO:

Introduction: The mental practice is considered a cognitive strategy that enables sequential mental training of a given motor task, without its physical execution. This practice has been used in the rehabilitation of various types of neurological disorders and has shown positive effects in motor planning and learning. Individuals with Parkinson's disease (PD) generally present neurophysiological, motor and cognitive disorders; therefore, once the disease progresses, external strategies are needed to help neuronal and motor organization. However, the effectiveness of mental practice associated with physical practice on brain and functional activity of these individuals is yet unknown. Objective: To investigate the effects of mental practice associated with physical practice of the gait on brain and functional activity of subjects with PD. Methods: This is a randomized, controlled, single-blinded clinical trial, with 40 individuals with PD, between 50 and 75 years, without cognitive impairment, in the moderate stage of the disease and with ability to generate mental image. All participants will be assessed for cognitive function level, physical disability, mental image sharpness, electroencephalographic activity, mobility, balance, kinematic gait variables (spatio-temporal: velocity and step length; angular joints: hip, knee and ankle), motor function and activities of daily living. Participants will be randomly assigned to the experimental group (EG) and control group (CG). The EG  performs the mental practice and physical practice of the gait, while the CG will hold only the physical practice. There will be 12 training sessions, with a maximum of 1 hour each, 3 times a week for 4 weeks. The individuals will be re-evaluated one day and seven days after the last training session, with regard to the pattern of brain activation, the mental image sharpness, mobility, balance and gait speed. Expected results: It is expected that the effects on the cerebral activation, mobility, balance and gait velocity variables, resulting from the mental practice associated with physical practice, show a better improvement than the effects resulting from the physical practice alone, and that there is a significant positive correlation of activation between these variables. Thus, this study will yield important information for the development of this treatment protocol, corroborating with the scientific evidence and clinical practice.
