RESUMO:

Histoplasmosis is a worldwide disease that affect mainly immunocompromised patients. In Brazil, the mortality rate of histoplasmosis may achieve up to 72%, mainly due the troubles in the diagnosis of this disease. The fungal culture is the choose method to diagnosis histoplasmosis, although may take up to 8 weeks to conclusive results. Histopathology, however, is unable to identify the infectious agent in the sample, although it is indicative of infection. The immunoassays have sensitivity influenced by immune status of the patient and clinical manifestations, and often cross-reacting with some other fungi or bacteria. Molecular methods have been applied in diagnosis of histoplasmosis. However, these methods are not available in many laboratories and require well-trained experts. The objective of this work was to develop a LAMP assay (Loop-mediated Isothermal Amplification) to detect H. capsulatum in clinical samples. Due to its isothermal character, this reaction does not depend of thermocyclers, allowing to analyze the result in an alternative way to the agarose gel. The LAMP assay has been developed to detect ITS region in the genome of H. capsulatum, acquiring good sensitivity (limit of detection of 1 fg/µl) and being able to detect all tested genetic variants. In addition, the LAMP assay was able to directly detect DNA extraction-free health blood spiked with yeasts of Histoplasma.
