SUMMARY:

Psychedelic-Assisted Psychotherapy (PAP) has demonstrated efficacy in the treatment of depression and Post-Traumatic Stress Disorder (PTSD), but its resource-intensive structure limits scalability. N,N-Dimethyltryptamine (DMT), characterized by rapid onset, short duration, and intense effects, offers potential for more time-efficient interventions; however, it requires specialized support to ensure safety and comfort. This open-label, dose-escalation study, registered at ClinicalTrials.gov (NCT05573568), evaluated a non-directive clinical support protocol for the administration of inhaled DMT in 27 healthy volunteers. The protocol included one preparatory session, a safety dose (5–15 mg) followed by a full dose (20–60 mg) on the same day, and four integration sessions, two on the dosing day, one the following day, and one 14 days later. DMT was administered using a medical-grade vaporizer. Integration tools included verbal report, psychometric questionnaires, and, optionally, mandala drawing. Semi-structured interviews conducted 28 days after dosing were analyzed using thematic analysis to assess participants’ perceptions of safety, comfort, and integration. Core themes emerged. Participants emphasized the importance of preparatory work, a supportive physical environment, consensual therapeutic touch, and symbolic tools such as mandalas in promoting safety and emotional processing. Inhalation was generally well tolerated, and the brief yet immersive DMT experience was considered manageable within the protocol. This study presents a feasible, scalable, and ethically grounded support protocol for the administration of inhaled DMT. By combining symbolic, psychological, and somatic strategies without in-session psychotherapy, it offers a potential alternative to resource-intensive PAP models and may inform future research and public health protocols.
