SUMMARY:

Introduction: Maternal mortality remains a serious public health problem, marked by social, economic, and health service access inequalities, especially in the postpartum period, which accounts for the majority of deaths and is still neglected by public policies and care practices. Hospital discharge without clear criteria, the absence of care transition strategies, and the fragmentation of the network contribute to the discontinuity of postpartum care, compromising the early detection of complications and the effectiveness of health promotion and prevention actions. Thus, it is essential to adopt strategies that promote articulation between levels of care, value welcoming, and recognize the postpartum woman as the central subject of care. Therefore, this study aims to contribute to this process, seeking to improve care in the postpartum period through the implementation of an integrated, effective, and user-centered care pathway. Objective: To implement the postpartum care pathway in a municipality in Northeast Brazil. Methodology: The intervention will follow the improvement cycle model, composed of the following steps: a) identification of the opportunity for improvement; b) analysis of barriers to shared discharge and compliance with protocols in primary health care; c) identification of the opportunity for improvement based on the perception of people in the postpartum period; d) quality assessment (baseline); e) planning and implementation of interventions to improve quality; f) quality reassessment (post-interventions); g) development and validation of the care pathway flowchart. The baseline will be obtained through standardized instruments applied to postpartum women with 42 days or more of follow-up, with a minimum sample of 60 women before and 60 after the interventions. Data will be collected via data collection instruments, structured interviews, and consultation of medical records and the pregnant woman's card, considering quality criteria for hospital discharge and follow-up by primary health care.
