SUMMARY:

Brazilian population aging, associated with the demographic and epidemiological transformations observed in recent decades, has increased the relevance of care needs in old age and highlighted challenges related to health conditions, functional capacity, and the possibilities for care provision. Changes in household arrangements and socioeconomic inequalities reinforce the need to understand care demand beyond individual health conditions, also considering the context in which care may be provided. In this context, this study aims to construct and describe the Multidimensional Index of Potential Care Demand for Older People (IMDPCI) for the Brazilian older population, integrating health and functional conditions with household and socioeconomic characteristics related to the potential context of care provision. This is a cross-sectional, quantitative study based on secondary data from the 2019 Brazilian National Health Survey (PNS), including individuals aged 60 years or older residing in Brazil. The IMDPCI will comprise two complementary dimensions. The Health Care Need (NCS) dimension will represent the individual dimension of care need and will comprise limitations in Activities of Daily Living (ADL), Instrumental Activities of Daily Living (IADL), and the burden of chronic diseases. The Vulnerability of the Care Provision Context (VPC) dimension will represent household and socioeconomic characteristics related to the potential capacity to respond to care needs and will comprise the Availability of Potential Adult Caregivers (DPCA), Household Care Composition (CDC), Competing Care Demands (DCC), and Socioeconomic Vulnerability (VSE). The components will be integrated using weights grounded in the conceptual framework and theoretical literature adopted in this study. In the final composition of the IMDPCI, a weight of 60% will be assigned to NCS and 40% to VPC, resulting in a synthetic measure expressed on an increasing scale from 0 to 3, in which higher values will indicate greater potential care demand. The distribution of NCS, VPC, and IMDPCI will be described according to sex, age group, race/skin color, educational attainment, and the Major Regions of Brazil, taking into account the complex sampling design of the 2019 PNS. The index is expected to contribute to a broader understanding of the heterogeneity of potential care demand among older people in Brazil by integrating aspects related to care needs and the conditions of the context in which care may be provided, thereby producing evidence that may support the planning and improvement of public policies aimed at the older population in the context of Brazilian population aging.
