SUMMARY:

Patients with difficult-to-heal wounds include those with wounds that have not healed 40-50% of their extension after four weeks of adequate clinical analysis and intervention. This health situation is related to biological, clinical, non- clinical and service delivery factors, and also requires the use of multiple services and levels of complexity in the health care network. Therefore, to reduce the impacts of this problem on public health, Transitional Care is essential, a care modality that seeks to improve the results associated with patients, their families, caregivers and health systems during the transfer between different units, services and levels of care. Thus, the present study aimed to analyze the management of Transitional Care for patients with difficult- to-heal wounds in the health care network after hospital discharge. This is a single integrated case study of a qualitative nature and its unit of analysis is the health care network of the municipality of Natal, Rio Grande do Norte. It was conducted according to the guidelines of Yin (2015). The sources of evidence used were semi-structured interviews with users with difficult-to-heal wounds who were discharged from hospital; with nurses and workers who work in the management or assistance of services that make up the health care network of the municipality under analysis; and a scoping review on the management of transitional care for users with wounds. The data obtained were analyzed using the content analysis method proposed by Bardin (2016) and through the philosophical framework of the Theory of Communicative Action, by Jurgen Habemas, with support from the ATLAS TI® software. The results were presented in texts, figures, tables, charts, in addition to the graph produced in the software and organized in the case study report. The preliminary results of this study indicate that the transition of care for users with difficult-to-heal wounds begins at hospital admission, with the participation of the multidisciplinary team, the user and caregivers. Although the necessary pillars for transitional care were observed, elements such as communication and coordination among the members of the multidisciplinary team were shown to be weak. These problems extend to other services in the health care network, which interferes with the continuity of care for these users and their recovery after hospital discharge. Furthermore, weaknesses in the health care network and elements related to the social condition of users also stood out as critical nodes for transitional care. In all health services that participated in the study, the participation of nurses as active professionals in the management of the process studied was evident. The scoping review identified strategies for managing Transitional Care based on the use of technologies and adjustments in work processes aimed at improving this process in health services. In these situations, the role of nurses in managing Transitional Care was prevalent.
