SUMMARY:

Introduction: Stroke remains among the leading causes of disability and mortality worldwide, requiring effective organization of care and rehabilitation, especially in the context of Primary Health Care (PHC). This study aimed to evaluate the organization of post-stroke rehabilitation in PHC through the application of the STRIDE questionnaire in municipalities in the Trairi and Potengi regions of Rio Grande do Norte. Methodology: This is a methodological, observational, cross-sectional, and descriptive study, developed in two stages: methodological evaluation of the instrument and application of the questionnaire in PHC services. Thirty-two professionals from the areas of physiotherapy, nursing, and speech therapy participated, working in Basic Health Units, multidisciplinary teams, and Home Care Services. The data were analyzed using descriptive statistics using IBM SPSS Statistics 23.0 software. Results: It was observed that nurses and physiotherapists predominated, with low specific qualifications in neurological rehabilitation and reduced participation in training focused on stroke. Weaknesses were found related to the absence of structured protocols, limited use of standardized assessment instruments, insufficient professionals, the presence of waiting lists, and low availability of assistive technologies. Care practices showed heterogeneity among professional categories, with greater systematization in speech therapy, consistent functional performance of physiotherapy, and relevant participation of nursing, although with limitations in continuing education and in the structuring of specific teams for rehabilitation. Weaknesses were also identified in the articulation between the levels of care, with a predominance of informal referral flows and a low offer of educational actions for family members and caregivers. Conclusion: Post-stroke rehabilitation in primary health care presents significant limitations in the organization of services and the integration of the care network, highlighting the need to strengthen multidisciplinary teams professional qualification and implementation of evidence-based protocols, aiming to expand access, comprehensiveness and quality of care for stroke patients in the Unified Health System.
