SUMMARY:

Introduction
Persistent Spinal Pain Syndrome Type 2 (PSPS-2) is a complex
clinical condition characterized by the persistence of spinal and/or radicular
pain following spinal surgery, with substantial functional, psychosocial, and
socioeconomic impact. The updated terminology proposed by the
International Association for the Study of Pain (IASP) emphasizes the
multifactorial nature of this syndrome, involving interactions among
structural changes, neurophysiological mechanisms, functional impairments,
and psychosocial factors. In this context, rehabilitation-based strategies
grounded in the biopsychosocial model have become central to
management; however, the existing literature remains fragmented and
methodologically heterogeneous, limiting the consolidation of evidence and
the development of standardized clinical recommendations.
Objectives
This scoping review aims to map and synthesize rehabilitation-based
interventions used in the management of PSPS-2 and the clinical and
functional outcomes employed to evaluate these patients, while describing
the characteristics of the studied populations, intervention protocols, and
existing gaps in the literature.
Methods
A scoping review will be conducted in accordance with the Joanna
Briggs Institute methodology and the PRISMA Extension for Scoping
Reviews (PRISMA-ScR). Studies involving adult patients with PSPS-2
undergoing rehabilitation-based interventions—such as therapeutic exercise,
physical therapy, cognitive behavioral therapy, pain education, and
multimodal programs—will be included. Systematic searches will be
performed across multiple electronic databases and gray literature sources,
without restrictions on language or publication date. Study selection, data
extraction, and synthesis will be carried out independently by reviewers,
with results presented in a descriptive and structured manner.
Expected Results
It is expected that a wide range of rehabilitation-based interventions
will be identified, with a predominance of multimodal programs combining
therapeutic exercise and psychosocial approaches. Anticipated findings
include modest to moderate improvements in pain, physical function, and
quality of life, with more consistent effects observed in functional and
psychosocial outcomes than in pain intensity alone. Considerable
heterogeneity in intervention protocols, outcome measures, and follow-up
periods is also expected, along with important methodological gaps such as
limited long-term follow-up and lack of standardized outcome reporting.
These findings are expected to inform future research directions and support
the development of more consistent, evidence-based rehabilitation strategies
for patients with PSPS-2.
