SUMMARY:

Background: The COVID-19 pandemic, caused by the SARS-CoV-2 virus,
posed significant challenges to healthcare professionals worldwide, who faced
high levels of stress and risk of contamination. The prolonged use of Personal
Protective Equipment (PPE), coupled with an intense workload, resulted in an
increase in reports of craniocervical symptoms in this population. Evidence
suggests that a combination of physical and psychological factors plays a
crucial role in the high prevalence of these symptoms. However, the
relationship between these factors and other craniocervical symptoms beyond
headache, such as neck pain, orofacial pain, and temporomandibular
dysfunction, still requires further investigation. Objective: To investigate the
prevalence of craniocervical and orofacial symptoms among healthcare
professionals during the COVID-19 pandemic, identifying potential factors
associated with the onset, development, progression, and aggravation of these

symptoms in this context. Methods: A systematic review was conducted
following PRISMA guidelines, with a protocol previously registered in
PROSPERO. The review included observational studies published since 2020
that reported the prevalence or incidence of craniocervical symptoms, such as
headache, neck pain, and orofacial pain, in healthcare professionals during the
pandemic. Searches were conducted in the PubMed, MEDLINE, EMBASE,
CINAHL Plus, Web of Science, and SCOPUS databases, encompassing
studies from all continents. No language restrictions were applied. Partial
results: Thirty-seven studies were included, totaling 15,086 healthcare
professionals, including physicians, nurses, physiotherapists, and others. The
prevalence of headache among these professionals during the pandemic was
41.5%. Individual, medical, and environmental factors were associated with the
occurrence of headache in the context of COVID-19. Neck pain was the second
most evaluated craniocervical symptom, followed by orofacial pain.
Additionally, the workplace, workload, use, and type of personal protective
equipment (PPE) were also analyzed, along with the presence of anxiety,
depressive symptoms, stress, fatigue, and sleep disorders. These findings
highlight the importance of addressing the factors that contribute to the high
prevalence of craniocervical symptoms among healthcare professionals.
