SUMMARY:

Introduction: Duchenne Muscular Dystrophy (DMD) is a rare genetic disease
linked to the X chromosome, caused by mutations in the dystrophin gene.
Over time, it progresses with deformities of the thoracic spine and ribs, in
addition to progressive weakness of the diaphragm and accessory respiratory
muscles, leading to hypoventilation, nocturnal desaturation, and ineffective
coughing. Ineffective coughing can lead to severe respiratory complications.
Currently, there are several techniques for assisted coughing, and three of
them can be highlighted: non-invasive ventilation, air stacking, and
mechanical insufflation-exsufflation. Methodology: This is a cross-sectional
experimental crossover study conducted at the Pneumocardiovascular
Laboratory of the Onofre Lopes. Objective: To evaluate the acute effects of
three assisted coughing techniques in individuals with Duchenne Muscular
Dystrophy.University Hospital (HUOL) - EBSERH/UFRN. The study will
include individuals diagnosed with DMD, aged between 12 and 20 years, who
have been using non-invasive ventilation (NIV) for at least six months; who
use NIV daily for at least 4 hours a day; who tolerate the use of a nasal mask;
who have sufficient cognitive level to understand the procedures to be
performed; who are free of associated infectious pulmonary diseases; and
who are not tracheostomized. The data collection procedures will include a
clinical assessment, evaluation of pulmonary function and respiratory muscle
strength, peak cough flow assessment, and evaluation of the
electromyographic activity of the respiratory muscles. The intervention
protocol will involve three assisted coughing techniques: non-invasive
ventilation, air stacking, and mechanical insufflation-exsufflation. For
statistical analysis, the Shapiro-Wilk test will be used to assess data
normality, and descriptive analysis will be presented according to whether the
data is parametric or not. For comparisons, one-way ANOVA or a similar non-
parametric test will be used. The data will be analyzed using the GraphPad
Prism 6.0 statistical software. A significance level of p<0.05 and a 95%
Confidence Interval (CI) will be considered. Expected Results: It is expected
to contribute to the knowledge and application of the three different
techniques for assisted coughing, aiming to improve the treatment of
individuals with neuromuscular diseases and identify whether one of the
techniques is superior to the others and if there is any difference between
them.
