SUMMARY:

Introduction: Diabetes Mellitus (DM) is considered a public health problem. It is estimated that 90 to 95% of DM cases are type 2, with a greater presence in developing countries. Type 2 Diabetes Mellitus (DM2) evolved from rapid cultural, economic and social changes. Many people with T2DM are obese or overweight. The increase in physical inactivity and the number of illnesses results in a substantial loss of bone mineral density, while body fat is preserved or increased. With the global obesity epidemic, low bone mineral density and an increase in DM are expected in the coming years. In this context, the measurement of variables and their study are essential in the management of these disorders, in addition to interventions aimed at achieving adequate bone mineral density, which can help control DM2. Objective: To evaluate the discriminative validity and diagnostic accuracy of gynoid fat (%) using Dual-energy X-ray absorptiometry (DEXA) in body composition among women with and without DM2; and evaluate individuals with and without DM2 in relation to bone mineral density and sociodemographic, clinical, functional and laboratory variables. Methodologies: methodological study of discriminative validity and diagnostic accuracy of gynoid fat (%) carried out through a cross-sectional clinical study; and a cross-sectional analytical observational study in which individuals of both sexes were evaluated with a minimum age of 40 and a maximum of 85 years. The instruments applied were: clinical characterization questionnaire, cognition (Mini-Mental State Exam); level of physical activity (International Physical Activity Questionnaire); body composition and bone mineral density (DEXA). Results:Article 2 - A total of 130 women were evaluated, of which 60 (46.2%) had a clinical diagnosis of DM. When comparing groups, the percentage of gynoid fat showed a statistically significant difference (U = 1484; p = 0.004). The effect size found was 0.25. A cutoff point ≤ 47.90 was identified. Article 3: A total of 168 individuals were evaluated, 83 with DM2 and 85 without DM2. Females were the majority of both groups with and without DM2. The mean age of individuals with DM2 was 62.78 years ± 8.84 and of individuals without DM2 it was 57 years ± 9.97. The predominant skin color in both groups was white. The median level of education, number of diseases and HGS was very similar in both groups. Regarding the practice of physical activity, most individuals with DM2 were sedentary, while the majority of individuals without DM2 practiced physical activity. Individuals with DM2 used more medications compared to the group without DM2. For individuals with DM2, the median time since diagnosis was 3.5 years (0-60). Comparing glycated hemoglobin in the 2 groups, it was observed that the median was higher in the group with DM2. The majority of individuals in both groups reported feeling pain in their lower limbs and not having fallen in the last 6 months. Conclusion: Article 2 - The group of women with DM2 had a lower percentage of gynoid fat and the cutoff point indicated women with DM2 from gynoid fat.
