SUMMARY:

The different degrees of prematurity have distinct repercussions on infant health. A lower gestational age at birth is associated with greater clinical and nutritional vulnerabilities, directly influencing the approach adopted during complementary feeding. Antioxidants are micronutrients capable of combating the production of free radicals, which are increased in premature infants due to fragile antioxidant defenses and immature body reserves, thereby raising the risk of developing morbidities. The aim of this study was to analyze the intake of antioxidant micronutrients (vitamins A, C, E, zinc, and selenium) during complementary feeding in premature infants according to the degree of prematurity, and to associate them with anthropometric and sociodemographic variables. This was an observational, cross-sectional, and analytical study conducted at the Januário Cicco Maternity Hospital. Variables collected included the degree of prematurity, intake and adequacy of antioxidant intake, percentage of infant formula, clinical-anthropometric profile (at birth and at complementary feeding initiation), breastfeeding habits, and dietary practices of premature infants, gathered through medical records, anthropometric measurements, semi-structured interviews, and 24-hour dietary recalls. Statistical analyses included descriptive statistics, Student's t-test/Mann-Whitney test, and chi-square test for group comparisons, EAR cut-off point method, principal component analysis, and linear regression for associations, with a significance level set at 5%. The results showed a mean micronutrient intake that did not differ significantly between groups. The contribution of infant formulas was high, particularly for vitamin E. Although minimum dietary diversity was achieved by the majority of infants, the prevalence of inadequacy remained high, especially for vitamin E and selenium, but was low for vitamins A and C. These results reinforce the need for individualized nutritional guidance and caregiver education regarding the choice and progression of complementary feeding, as well as multiprofessional follow-up, particularly for those with lower gestational age, to ensure an adequate supply of nutrients tailored to their age and increased nutritional needs.
