SUMMARY:

Introduction: Human papillomavirus (HPV) is responsible for approximately 70% of cervical cancer cases. Vaccination has proven to be an effective prevention strategy. However, the efficacy and safety of a single-dose regimen compared to multiple doses remain uncertain. Objective: To evaluate the safety and efficacy of single-dose HPV vaccines compared to multiple-dose regimens or no vaccination. Methods: We systematically searched (insert databases here) for randomized controlled trials (RCTs) investigating the safety and efficacy of a single dose of HPV vaccine in healthy women. Risk of bias assessment (RoB2) was performed by independent reviewers. Results: Five RCTs with 3,401 participants were included. The single-dose regimen showed a favorable safety profile, with local pain, fatigue, and headache as self-limiting adverse events. Bivalent and nonavalent vaccines showed sustained protection against infection by types 16 and 18, with long-lasting seropositivity. No serious adverse events were attributed to the vaccination. The efficacy of the single dose was comparable to that of two- or three-dose schedules, regardless of the vaccine type. Conclusion: Single-dose HPV vaccination demonstrated sustained safety and efficacy, representing a viable strategy to optimize immunization programs, especially in low- and middle-income countries.
