SUMMARY:

The management of financial resources within Brazilian public health is characterized by structural budget constraints, increasing demand for services, and legal rigidity regarding expenditure execution. In this context, the insufficiency of resources to promptly settle obligations compels public managers to prioritize payments, including in situations involving exceptions to the chronological order established by current legislation. Given this scenario, this dissertation aims to develop a multicriteria decision support model to assist in the prioritization of payments within a State Department of Public Health, considering budget limitations and multiple decision criteria.

The research is classified as applied, exploratory, and descriptive in nature, and is conducted through a case study. Methodologically, it integrates Problem Structuring Methods (PSM) and Multi-Criteria Decision Aiding (MCDA). Initially, the Value-Focused Thinking (VFT) approach is employed to identify, structure, and hierarchize the decision maker’s objectives and values, enabling the construction of a consistent set of evaluation criteria. Subsequently, the FITradeoff method is applied to model preferences and prioritize payment alternatives, allowing the simultaneous consideration of criteria such as healthcare impact, risk of service discontinuity, legal implications, urgency, and financial constraints. The model is complemented by sensitivity analysis to assess the robustness of the results.

The findings demonstrate that the proposed approach contributes to greater transparency, rationality, and consistency in the decision-making process, reducing subjectivity and cognitive effort for managers in highly complex scenarios. Furthermore, the model presents potential for replication in other public administration bodies facing similar challenges. From an academic perspective, the study expands discussions on the application of multicriteria methods to the management of public health payments, a topic that remains underexplored in the literature.
