SUMMARY:

Objective: To systematize Permanent Health Education (PHE) initiatives for Community Health Agents (CHWs) to reorient practices and strategies for fall prevention among older adults in the PHC setting. Methodology: This is an interventionist, qualitative research study developed through action research. It was conducted in the city of São José do Sabugi, Paraíba. The research targeted CHWs who work in Primary Health Care (PHC) in this municipality. Because this is action research, the methodological approach was systematized according to the interconnected phases that comprise the method. In the Exploratory Phase, CHWs were invited to participate in the study and were informed about the research, its objectives, course, and duration through a meeting/seminar. In this phase, data were collected through an Investigative Questionnaire. In the Planning Phase, the researcher planned the educational actions to be implemented with the CHWs. The Action Phase involved the execution of the planned actions, focusing on three educational meetings through EPS workshops. At the end of this phase, a workshop evaluation instrument was administered. The Evaluation Phase involved data analysis (Investigative Questionnaire and Evaluation Instrument) using simple descriptive statistics. The narrative analysis of the workshops was supported by field diaries and discussions based on related literature. Finally, an illustrative booklet entitled "Fall Risk Prevention and Identification in the Elderly" was created. Results: The Continuing Health Education strategy, conducted using active methodologies and focused on the needs of the region, was effective in improving the knowledge of CHWs about fall prevention, promoting the redefinition of practices in daily work, encouraging the use of tools such as the Elderly Health Handbook (CSPI) to monitor the health of older adults, and strengthening the autonomy of CHWs in developing educational initiatives in the region. Final Considerations: This work contributes significantly to strengthening public health policies, especially regarding older adult care in Primary Care. It also contributes to the advancement of applied scientific knowledge, promoting the integration of practice, teaching, and research in the field of public health and aging.
