Introduction: Among the methods used to investigate Urinary Incontinence (UI), the bladder diary (BD) is one of the most utilized tools for diagnosing lower urinary tract symptoms (LUTS), typically used for three days. The International Consultation on Incontinence Questionnaire – Bladder Diary (ICIQ- Bladder Diary) is a BD that was recently translated and validated for Brazilian Portuguese. However, the literature lacks an evaluation of the cutoff point for female LUTS analyzed by the ICIQ-BD that can discriminate clinical improvement. Objectives: To determine the cutoff point for female urinary symptoms assessed by the ICIQ- Bladder Diary to identify clinical improvement. Material and Methods: This was a multicenter study with secondary data and a diagnostic accuracy methodological approach. The study included Brazilian women over 18 years old who could read and write in Portuguese, had complaints of UI in the three months prior to the study, and had pelvic floor muscle strength (PFM) >1 on the Modified Oxford Scale. Exclusion criteria included pregnant women, those within six months postpartum, women with lower urinary tract infections at the time of evaluation, interstitial cystitis, current or previous urogenital neoplasms, neurological diseases, and those who had received treatment for pelvic floor muscle dysfunction in the three months prior to the study and between the first and second evaluations. Three assessments were conducted. During the first assessment, participants completed an anamnesis covering sociodemographic data, gynecological and obstetric history, lifestyle habits, and questionnaires, including the Questionnaire for Urinary Incontinence Diagnosis (QUID), Incontinence Severity Index (ISI), Global Perception of Improvement Scale (GPIS), and the ICIQ-BD for three days, as well as an evaluation of PFM. The second assessment was conducted 15 days later, where participants completed the ICIQ- Bladder Diary again for one day, followed by eight weeks of PFM training. The final assessment included completing the GPIS and the ICIQ-BD for one day. Results: Fifty women completed the study. The mean age was 44.7 (12.4) years, and the mean BMI was 28.2 (5.2) kg/m². ROC curve analysis identified that a reduction of one urinary leakage episode (AUC=0.78), two fewer voiding episodes per day (AUC=0.9111), and one fewer nocturia episode (AUC=0.880) were sufficient for participants to report clinical improvement. Conclusion: The ICIQ- Bladder Diary was able to detect the participants' reported sense of clinical improvement. This discrimination was moderate for a reduction of one UI episode, excellent for a reduction of two voiding episodes, and good for a reduction of one nocturia episode.
