Diabetes mellitus (DM) is a endocrine disease, defined by high concentration of blood glucose (hyperglycemia) marked by metabolic abnormalities and diffuse long-term complications. One proeminent diabetic complication is diabetic nephropathy (DN), characterized by podocyte loss and increase of mesangial matrix, leading to pronounced proteinuria. Currently, regular exercise has been considered one of the main approaches in DM treatment. A scant number of studies have reported that exercise therapy may delay the progression of kidney disease. Some studies, however, indicate that acute exercise may intensify some diabetic alterations since it causes additional oxidative stress. Thus, the main aim of this study was to evaluate the effect of moderate aerobic exercise at morphofunctional, biochemical and molecular effects of previous exercise training in renal tissues of diabetic animals. We hypothesized that a previous period of exercise (before DM onset and exercise therapy) could prevent excessive oxidative damage occurring when exercise therapy is initiated after DN is in progress. Male Wistar rats, 30 days old, will be assigned into the following groups (n = 15 / group): sedentary control (SC), trained control (TC), sedentary diabetic (SD), trained diabetic (TD) and previously trained diabetic (PTD). Diabetes will be induced by streptozotocin injection (60 mg/kg, ip). At the confirmtion of DM onset (blood glucose > 250 mg/dL), rats of TC, TD and PTD groups will undergo a six-week swimming program (30 min/day, 3 days/week). The PTD group will be also submitted to the same intensity exercise regimen in the previous four weeks in relation to DM confirmation. The blood sample was taken for biochemical analysis (glucose, serum creatinine and albumin). Kidneys will be collected for histological analysis of the renal parenchyma integrity (hematoxylin and eosin), fibrotic tissue formation (picrosirius red), thickening of the glomerular basement membrane (periodic acid-Schiff). Beside, podocyte density (immunohistochemical staining and Scanning electron microscopy). The activity of antioxidant enzymes superoxide dismutase (SOD), catalase (CAT) and glutathione peroxidase (GPX), as well as levels of lipid peroxidation by the reaction of thiobarbituric acid (TBARS) and protein peroxidation (carbonyl groups - DNPH) will be also measured. Finally, gene expression (RT-qPCR) of the following mesangial matrix proteins will be performed: collagen types I and IV, fibronectin and podocin. Animals diabetic group had a higher glycemic index when compared to control groups (p <0.05). There was also an improvement in the trained diabetic groups when compared to sedentary diabetic group, but was not significant. Creatinine was increased in all groups compared to control (p <0.05). Albumin, and the weight was decreased in diabetic group compared to the control group (p <0.05). DM resulted in a kidney hypertrophy in diabetic groups compared with control groups (p <0.05). As an increase in size and number of glomeruli of the diabetic groups compared to control groups (p <0.05). Partial results are still inconclusive due to lack of analysis. However, with this research, we hope to contribute to the understanding of mechanisms related to oxidative stress and exercise in DN, and provide basis for future clinical and experimental studies involving exercise as therapeutic approach for kidney protection in diabetic state.
