Toxoplasmosis is a zoonosis present throughout the world with the ability to infect any homeothermic animal, including humans. This disease is particularly serious in immunocompromised and pregnant women. Primary infection in pregnant women is usually asymptomatic and the parasite may cross the placenta, infecting the fetus, causing retinochoroiditis, hydrocephalus, mental retardation, convulsions, or even fetal death. Due to the importance of toxoplasmosis in public health, this study aimed to estimate the incidence and prevalence of infection in pregnant women in the city of Santa Cruz (RN), as well as to investigate the factors associated with this infection, including individual, behavioral and socioeconomic factors. In this study, 178 pregnant women were approached and the serological results for Toxoplasma gondii specific IgM and IgG antibodies were obtained by ELISA and Electrochemiluminescence. The incidence was found to be 0.6% and the prevalence was 28.7% among the pregnant women attended at the Basic Health Units of the city, showing that 71.3% of the women are still susceptible to infection. The neighborhoods of Maracujá and Paraiso presented higher number of pregnant women with positive IgG serology when compared to the other neighborhoods. When only women with negative (susceptible) IgG serology were observed during the first gestation and the younger women, it was verified that these two groups presented in greater number, characterizing vulnerability to infection during a possible later pregnancy. We observed that professions/occupations that possibly expose them continuously to risk factors for illness and habits such as underdone meat consumption, gardening practices, consumption of water directly from tap and salad without knowledge about the origin of hygiene are common practices among the evaluated pregnant women. The presence of a fully closed or partially closed water reservoir may be functioning as a protective factor for infection among pregnant women. Studies such as these can be widely used to support differentiated public policies, as they allow the recognition of vulnerable groups and their exposure to risk, thus contributing to disease prevention. In addition, it allows informing pregnant women and health professionals as to the current epidemiological situation of toxoplasmosis in the city.
