RESUMO:

INTRODUCTION: The use of invasive devices in certain medical procedures is one of the most important causes of infections acquired in the hospital environment, which represent a great risk to patients' health. This study evaluated the occurrence of infections transmitted by these instruments in patients treated at the Hospital Universitário Onofre Lopes. OBJECTIVE: Analyze all cases of healthcare-associated infections (HAIs) resulting from the use of invasive devices (central venous catheter, urinary catheters, mechanical ventilator) identified in an adult ICU between January 2016 and December 2017. METHODOLOGY: This is a descriptive, retrospective and epidemiological study. The data were collected in Microsoft Office Excel spreadsheets from the HUOL database and the variables: gender, age, average length of hospital stay, types of invasive procedures performed, cause of hospitalization, main site of infection, isolated microorganisms, clinical outcome of the patient, profile of antimicrobials used and resistance were analyzed by statistical tests (Test Qui-Quadrado, Spearman, Friedman). RESULTS: Through the notifications from the CCIH, it was verified that 130 cases of infections were identified, 44.6% in 2016 and 55.4% in 2017. Male and female patients were also affected and the mean age found was equal to 58.8 years. There was an increasing density in the occurrence of infections in older age groups in cases involving patients aged ≤ 45 and <75 years. The devices most used were urinary catheter, central venous catheter and mechanical ventilator, in order and the most observed type of infection were pneumonias associated with mechanical ventilation. The isolated microorganisms belong mostly to the group of Gram-negative bacteria, with a higher prevalence of the following bacterial species: Pseudomonas aeruginosa, Klebsiella pneumoniae e Acinetobacter baumannii. The most prescribed antimicrobial was the carbapenem meropenem. No statistically significant associations were found between the use rate of dispositive and the incidence density of each type of infection or between the variables "age, length of hospital stay, number of devices" versus death. CONCLUSION:An expressive prevalence of infections transmitted by invasive devices, mainly caused by gram-negative bacteria, was observed, mainly reaching older patients, and pneumonia associated with mechanical ventilation was the most frequent clinical manifestation.
