RESUMO:

Toxoplasmosis is caused by the obligate intracellular parasite Toxoplasma gondii. In humans, toxoplasmosis is asymptomatic or self-limited in immunocompetent, however, in immunocompromised patients the disease develops in a more serious way, relevant to the demonstrations in the central nervous system, and the neurotoxoplasmosis is the predominant focal brain injury in patients HIV positive. This study aimed to assess the frequency of T. gondii infection in HIV-positive patients and to correlate the clinical, serological and epidemiological. For this, 179 HIV-positive patients treated at Hospital Giselda Trigueiro were evaluated. Serological analyzes were performed by ELISA, while the clinical and epidemiological analysis occurred through research on medical records and questionnaires, respectively. Moreover, molecular analysis was performed using PCR in peripheral blood samples from 11 HIV patients hospitalized with neurotoxoplasmosis (suspected or confirmed). From this study, it was observed seroprevalence of 83.8% of T. gondii infection in HIV patients. In the analysis of risk factors, it was found that the risk of acquiring toxoplasmosis was higher among patients who reported the neighborhood cats from coming into their homes (OR = 2.8; p = 0.04) and among those who eat raw salad away from home (OR = 3.6; p = 0.00). Was found that 11.7% had neurotoxoplasmosis and 3.35% ocular toxoplasmosis. Furthermore, it was found that the CD4 lymphocyte levels of 179 patients analyzed had an average of 580.2 cells / mm³, but 8.9% are at risk for development of cerebral toxoplasmosis. The most common clinical manifestations among all patients affected by cerebral toxoplasmosis were: paresis / hemiparesis (65.62%), headache (37.50%) and altered consciousness (28.12%). In addition, among patients HIV / AIDS admitted to neurotoxoplasmosis was due to reactivation of latent infection, so that 90.9% didn’t perform ART regularly. Molecular assessment presented limitations on the diagnosis of cerebral toxoplasmosis. We conclude that this work allowed an initial characterization of toxoplasmosis frame in HIV / AIDS in Rio Grande do Norte, being a pioneering study in the region.
