SUMMARY:

Introduction: The concept of a culture of care relates to mutual respect among workers, peer support, and institutional commitment to the well-being of professionals, directly influencing the quality of care, job satisfaction, and the sustainability of healthcare organizations. Despite its relevance to management and clinical practice, studies exploring this concept remain limited, and to date, there is no validated instrument for assessing the culture of care in the Brazilian context. Therefore, the objectives of this study were to examine how the concept of culture of care has been addressed in the scientific literature and to conduct the cross-cultural adaptation and psychometric validation of the Culture of Care Barometer instrument into Brazilian Portuguese. Methods: This study consists of two complementary methodological designs: a scoping review (studies 1 and 2) and a methodological study on the cross-cultural adaptation and psychometric validation of the instrument (studies 3 and 4). The scoping review followed the methodological recommendations of the Joanna Briggs Institute (JBI). Searches were conducted in the PubMed, LILACS, Cochrane Library, and Embase databases, including studies addressing the concept of the culture of care in health services, available in full text, with no restrictions on language or publication period. The methodological study isbeing conducted in two phases: cross-cultural adaptation of the instrument and psychometric validation. The cross-cultural adaptation process included translation, synthesis, back-translation, expert review and pre-testing, based on Beaton’s model (2000). The psychometric validation phase is the next step to be developed in this study. Results: In the scoping review, the search strategy initially identified 2,506 references, of which 7 were included in the study. Additionally, through a manual search of reference lists, another 8 studies were identified, of which 5 met the inclusion criteria, for a total of 12 studies analyzed. The results of the review indicate that the literature on care culture focuses on two main areas: (i) the development and validation of instruments designed to measure care culture in different organizational contexts; and (ii) conceptual reflection on the centrality of care in healthcare organizations. The results of the methodological study, meanwhile, show high agreement among experts for most items, with a need for specific adjustments to ensure terminological and cultural appropriateness for the Brazilian context. The pre-test of the pre-final version, conducted with 30 health service managers and leaders, revealed high levels of understanding and relevance of the items, indicating adequate clarity, semantic equivalence, and applicability of the instrument in the Brazilian context, with the need for some adjustments to the wording of certain items. Partial considerations: It is hoped that the Brazilian version of the Culture of Care Barometer will contribute to strengthening research and practices aimed at assessing the culture of care in health services, thereby supporting strategies for workforce management, promoting healthier organizational environments, and improving the quality of care.
