INFECTIONS RELATED TO HEALTH CARE IN HOSPITALS IN THE STATE OF AMAZONAS: AN ECOLOGICAL STUDY

Introduction: Healthcare-Associated Infections (HAIs) are considered a serious public health problem and relevant adverse events that can interfere with patient safety. Objective: To understand the HAIs in hospitals in the State of Amazonas from 2018 to 2022. This is an ecological, descriptive, exploratory, retrospective study with a quantitative approach. Methods: Data from all reporting hospitals that have adult ICU beds and that perform cesarean delivery, breast implant, primary knee arthroplasty, primary hip arthroplasty, internal neurological bypass, and coronary artery bypass graft surgery were included in the study. Incidence Densities (ID) of HAIs of Ventilator-Associated Pneumonia, Primary Laboratory Bloodstream Infection, Catheter-Associated Urinary Tract Infection and also Surgical Site Infection (SSI) rates for surgeries with mandatory national notification were calculated. Trend analysis was performed using the Joinpoint program and presented in the form of graphs and tables. Results: Among the ICU HAIs, VAP had the highest number of infections in the period (14‰), followed by CLABSI (4.5‰) and AC-UTI (3.4‰). Regarding ICU HAIs, VAP, despite presenting a higher DI, a reduction was observed over time. In 2018, the DI was 16‰, in 2022 it reduced to 13.3‰. AC-UTIs also showed a reduction, a DI of 6.4‰ was observed in 2018, reducing to 2.5‰ in 2022. CLABSI was the one that showed an increase in DI, in 2018 the DI was 4.8‰, in 2022 it rose to 5.2‰. Regarding SSI, the surgery with the highest rates was myocardial revascularization (9.7%), followed by internal neurological bypass (5.6%), primary knee arthroplasty (3.2%), cesarean section (2.8%), primary hip arthroplasty (1%) and breast implant with 0.8%. From 2018.1 to 2021.1 there was a reduction of -0.36% in the APC. However, from this date until 2022 there was an increase of 2.38%, the only data that presented statistical significance (p<0.05). Therefore, cesarean section and primary knee arthroplasty surgeries, over time, had an increase in infection rates, cesarean section that in 2018 was 2.9%, in 2022 presented a rate of 3.4%, as for the primary knee arthroplasty procedure that in 2018 was 3.8%, in 2022 it went to have a rate of 4.4% of SSI. The other procedures showed a reduction in rates, myocardial revascularization, despite having the highest rates, went from 2018 with a rate of 15.6% to 2022 with a rate of 6.9%. Internal neurological bypass remained at practically the same rate as in 2018, which was 3.7%, and in 2022 it was 3.8%. Primary hip arthroplasty in 2018 had a rate of 2.7%, which reduced to 1.7% in 2022. Breast implant surgeries had low rates, in 2018 it was 1.9%, reducing even further in 2022 to 0.4%. Final considerations: In addition to harming patients, HAIs can increase hospital costs, harming healthcare as a whole due to the scarcity of resources, especially in the public health system. To prevent HAIs and possibly reduce rates, measures such as bundles, training of multidisciplinary teams, and campaigns to increase adherence to hand hygiene by healthcare professionals can be strategies allied to prevention.
