SUMMARY:

Introduction: Tuberculosis (TB), despite being an ancient disease, remains a significant public health issue in Brazil and worldwide. The disease results from infection by Mycobacterium tuberculosis, also known as Koch's Bacillus, and primarily affects the lungs, although it can also compromise other organs. Older adults are more susceptible to developing the infection due to the progressive decline in physiological systems inherent to the aging process. Objective: This study aims to analyze the associated factors, temporal trends, and scientific evidence regarding TB in older adults. Methodology: To achieve the proposed objectives, three research products were developed: (1) A retrospective, ecological, descriptive, cross-sectional, and quantitative study analyzing the temporal trends (TT) of TB incidence (TI) and mortality (TM) rates among older adults in the Brazilian Northeast from 2012 to 2022. Data collection was conducted through the Department of Informatics of the Brazilian Unified Health System (DATASUS). TT analysis was performed using the Prais-Winsten generalized regression model, which accounts for first-order serial autocorrelation (AR1). (2) A protocol for a scoping review aimed at identifying the main factors associated with pulmonary tuberculosis (PTB) in older adults. This protocol was developed based on the Joanna Briggs Institute (JBI) guidelines for this type of study and adhered to the recommendations of the Preferred Reporting Items for Systematic Reviews and Meta-Analysis Protocols (PRISMA-ScR) for scoping reviews. The protocol was registered in the Open Science Framework (OSF) with the following DOI: 10.17605/OSF.IO/DHQVP. (3) A scoping review currently in the finalization phase, which seeks to systematically and transparently map the state of the art regarding the key factors associated with PTB in older adults. Results: Article (1) demonstrated that both TI and TM were higher among male individuals, those of mixed-race/ethnicity, and those aged 60 to 69 years. Additionally, an increase in the Annual Percentage Variation (APV) of TB incidence was observed among females in the state of Pernambuco. The results of Article (2) comprise the protocol for the scoping review, which will serve as a guideline for the completion of the scoping review (3), currently in the phase of data synthesis and discussion. Final Considerations: This study is expected to provide evidence that can support more effective strategies for the prevention, diagnosis, and management of TB in this population. The findings highlight the need for specific approaches for older adults and foster a critical reflection on the challenges and advancements in the care of this vulnerable group.
