SUMMARY:

Maternal health is an area of high public health interest and its main indicator is the Maternal Mortality Rate. High maternal mortality rates are a reality in underdeveloped and developing countries, including Brazil. Among the socioeconomic characteristics, social inequality is one of the main conditioning factors of maternal death, since, in relation to health services, many of the actions of interest that impact on maternal death are part of the list of Primary Care responsibilities, such as planning family and prenatal care, being also essential the articulation between the care services and the quality of the care provided in childbirth and the puerperium. In this sense, in Brazil, the Stork Network has been in force since 2011, which has among its objectives the reduction of maternal mortality. In this context, the general objective of this research is to evaluate maternal mortality in Brazil before and after the implementation of the Stork Network. For this, an ecological study was carried out with analytical character of the periods 2005-2010 and 2012-2017, with data from secondary sources of 5565 cities of Brazil analyzed in five different levels: municipality, intermediate region of urban articulation, federation unit, region and Brazil. For data storage and processing, the software Microssoft Office Excel, Statistical Package for Social Science and Geoda were used, through which graphs and maps were constructed, and statistical tests were performed, allowing data analysis through descriptive and inferential statistics, including spatial analysis. The research complies with the ethical criteria advocated by Resolution 510/2016 of the National Health Council. The research results show that there was an increase in the average maternal mortality coefficient from 5.99 to 6.28 per 10,000 born, from the period before to the period after implementation of the Stork Network. Correlations between maternal mortality before and after the implementation of the Stork Network, with socioeconomic and demographic variables were weak, but all of them were statistically significant (p <0.05), and for both periods there was a negative correlation with the Human development Index, % of households with piped water, Life expectancy at birth, Aging rate and % of people aged 18 or older with completed high school, and positive with Gini Index, unemployment rate, % of women who had children in age group 10 to 17 years and fertility rate. The spatial correlation between the Maternal Mortality Coefficient and the Adherence to the Stork Network reveals that all the years analyzed were statistically significant high-high and also low-high clusters in the Northeast and North regions, while in the Southeast and South regions only low-low and high-low clusters. In addition, it is noted that in all the years analyzed the spatial correlation of the CMM with the adhesion to the Stork Network was weak, and in the first 3 years this correlation was positive (2012 = 0.12; 2013 = 0.025; 2014 = 0.003) and in the last 3 years it was negative (2015 = -0.025; 2016 = -0.022; 2017 = -0.026). Regarding the health indicators of interest to the Stork Network, it was observed that the number of children on exclusive breastfeeding increased from 35.01 to 160.35 children on exclusive breastfeeding/1,000 born in 2017; the number of prenatal consultations per pregnant woman in Brazil increased from 1.03 in 2012 to 4.19 in 2017; regarding the proportion of pregnant women who started prenatal care in the first trimester of pregnancy, this proportion increased from 34% in 2012 to 82% in 2017. About Coverage of Primary Care due to Adherence to the Stork Network, It has been found that over the years, the coverage of primary care in the intermediate region of urban articulation has increased and, concomitantly, a larger number of intermediate regions of urban articulation have reached 100% adherence to the Stork Network by the cities that compose them.
