RESUMO:

Antimicrobial photodynamic therapy (PDT) has been suggested as adjunctive therapy to basic periodontal scaling and coronal root straightening (RACR) therapy. In vitro and in vivo studies have demonstrated superior efficacy when PDA is performed with the photosensitizer Chloroaluminum phthalocyanine (AlClFc) in association with nanocarriers. However, there are no clinical trials evaluating its application in chronic periodontitis (CP). Objective: To evaluate the effect of TFDA on periodontal clinical parameters and on the salivary levels of oxidative stress markers Glutathione (GSH) and Malondialdehyde (MDA) in patients with CP. Methods: This randomized, double-blind, split-mouth, longitudinal and prospective study evaluated 52 periodontal sites, divided into two groups: 40 in the test group (RACR + TFDA) and 12 in the control group (RACR only). Scraping was performed in a single session, and TFDA was applied only once, both at the baseline (T0). The periodontal clinical parameters - bleeding probing index [ISS], visible plaque index [IPV], depth of probing [PS] and clinical insertion level [NIC] - were evaluated before any treatment at T0, and after the times of 3 months (T3) and 6 months (T6). Similarly, saliva samples were collected at each study time of all patients for further laboratory analysis. Data were statistically analyzed using non-parametric Wilcoxon and Mann-Whitney tests, with a significance level of 5% being pre-established. Results: In the intragroup analysis, with the exception of IPV, there was a statistically significant reduction of ISS, PS and NIC in the analyzed sites, both in the test group and in the control group. In the test group, a significant reduction of ISS was observed between T0 (100) and T6 (50), with p = 0.013, with no statistically significant difference between T0 and T3. In the control group, there was a significant decrease between T0 (100) and T3 (0), and between T0 (100) and T6 (0), with a value of p = 0.002. In relation to PS, a significant reduction of 2mm after 3 months and of 1.5mm after 6 months was verified in comparison with the T0, both in the test group and in the control group (p <0.001). The NIC reduced 0.5mm after 3 months and 1.5mm after 6 months in the test group, and 1.5mm after 3 months and 1.0mm after 6 months in the control group, both with a value of p <0.001. In the intergroup analysis there was no statistically significant difference between the treatment groups for all clinical parameters observed during the proposed study times. Conclusion: Adjuvant TFDA to the RACR did not bring additional benefits to the clinical parameters evaluated after three and six months of follow-up.
