SUMMARY:

Arboviruses are a group of viral diseases transmitted by insects, among which mosquitoes are the most widely distributed vectors. Some arboviruses are poorly understood and, consequently, fewer studies are conducted on them. Among these is Oropouche fever, a disease first recorded in 1955 in a charcoal worker on the island of Trinidad, and that has affected the Northern region of Brazil since 1961. However, starting in 2024, it gained national and international visibility due to genomic reassortment events and anthropogenic activities that facilitate its expansion. This systematic review aims to analyze the main aspects of this arbovirus, guided by the following research question: what are the primary vectors, reservoirs, environmental, and anthropogenic factors associated with the transmission and epidemiological expansion of Oropouche fever? To answer this question, the PRISMA methodology was used to search for articles across four databases: PubMed, Web of Science, Scopus, and ScienceDirect. A total of 1,372 articles were found across the four databases. Duplicates, book chapters, books, review articles, editorials, and articles that did not address Oropouche or Culicoides were excluded, resulting in 196 articles divided into nine themes: anthropogenic actions that favored the spread of the disease, symptoms affecting the population, epidemiology of Oropouche up to the present day, vector competence studies, bioecology of Culicoides, prevention, vertebrate hosts in which the virus was detected, and treatments. The results show a broad understanding of the epidemiology and anthropogenic factors responsible for the expansion of the disease, as well as prevention methods divided into collective and individual control strategies. However, this study reveals research gaps in the other thematic axes, underscoring the urgent need for further studies.
