SUMMARY:

Although the endometrial microbiome is not yet well defined, it is mainly represented by bacteria and its composition appears to be associated with gynecological diseases, which may have an impact on obtaining and maintaining pregnancy, being of great interest for reproductive medicine. Imbalance of the bacterial microbiome (bacteriome) can be associated with problems such as infertility, miscarriages and failed in vitro fertilization (IVF) treatments. The characterization of the endometrial bacterioma is of fundamental importance for clinical understanding and Chapter 1 of this work presents a comprehensive description of all the methodologies necessary for this analysis, with the aim of facilitating reproducibility and allowing comparative studies. Factors such as ethnicity, clinical data, hormonal status, aseptic sample collection, tissue preservation at -80oC, DNA extraction techniques, sequencing and bioinformatics tools with updated databases to identify pathogens at species level are the main ones.  Chapter II presents a study on the prevalence of the endometrial inflammatory state (endometritis) in the luteal phase, during the embryonic implantation window, comparing women with unexplained infertility and healthy women. The evaluation was carried out using the methodologies most used in clinical practice: video hysteroscopy with targeted endometrial biopsy and histological study with immunohistochemical evaluation to search for plasma cells using the Syndescan-1 (CD138) marker. A higher prevalence of chronic endometritis was observed in women with infertility, and although there is still no well-established consensus on the criteria for defining chronic endometritis, hysteroscopy-guided biopsy associated with immunohistochemical analysis should be considered an important diagnostic tool. in the evaluation of women with unexplained infertility. The evidence of microbial patterns that may be related to the endometrial inflammatory state and the development of diagnostic kits for clinical investigation of women with infertility is a promising field for the adequate treatment of endometritis in the area of human reproduction.
