RESUMO:

Introduction:The increase in the elderly population in Brazil will result in an increase in this population seeking health services. These services need to guarantee the quality of care provided and patient safety, which involve coordinated actions to prevent damage caused by the healthcare processes themselves. LPP are considered the main preventable damages related to assistance in health services, resulting from the long hospitalization of patients, being associated with risk factors, clinical conditions and also with an increase in the population's life expectancy. Effective risk management for LPP ranges from the assessment of the risk itself to the prescription and execution of care in light of the assessed risk, thus ensuring that the care provided is efficient. Objective: Ensure the effectiveness of care provided to elderly patients at risk of PI admitted to the ICU through improvement cycles. Method: Quasi-experimental study, before and after, without a control group, with a quantitative-qualitative approach, through the application of an improvement cycle carried out in the adult ICU from September 2024 to April 2025, at Hospital Antônio prudente Natal , located in the city of Natal, State of Rio Grande do Norte. The level of quality of care provided was measured before interventions, using seven quality criteria. The interventions were divided into two strategic lines: consultative audit of the processes evaluated during the improvement cycle and awareness/training of the multidisciplinary team on the provision of adequate care and records of its implementation, where it is still being carried out. Results: In the opportunity for improvement found – fragility of care provided to patients at risk of PI – we observed the possible causes, namely: lack of adequate risk assessment, absence of nursing diagnosis referring to the assessed risk, absence of prescription of care based on the diagnosis found, lack of execution of the care provided as well as checking the care provided. Based on these weaknesses, we built 7 quality criteria, 3 for structure and 4 for process. In the initial assessment, 30 cases were analyzed, where we observed, using the Pareto diagram, that the accumulated frequency of non-conformities resulted in a total of 82.14%, related to criteria 6, 7 and 4, both process criteria. As a result, action plans were drawn up along two strategic lines: training and auditing Conclusions: The improvements made so far show the significant weakness in the appropriate prescription by the nurse of care to the detriment of the assessed risk and the provision of care by the nursing technician, as well as its checking in medical records, and also the importance of raising awareness among teams regarding concerns the effectiveness of care provided in medical records. Interventions are still being carried out for improvement. With this study, it is expected to strengthen the safety culture in the institution, through best practices included in the service routine, involvement of the multidisciplinary team, effectiveness in managing the risk of PI and mainly improving the quality of care for elderly patients. admitted to the ICU, seeking a positive social impact.
