Background and aims: Vitamin A is essential for growth and development; however, preterm newborns have low reserve of vitamin A at birth, increasing the risk of morbidity and mortality. In the absence of supplementation, breast milk is the only source of this nutrient for exclusively breastfed infants. The main goal of this study was to assess the retinol concentration in serum and in the different phases of milk secretion in mothers of preterm newborns (<37 weeks), and check its adequacy to vitamin A nutritional requirements of this infants.

Methods: Blood was collected from 58 postpartum women. Milk was collected during colostrum (1-3d), transitional milk (7-15d), and mature milk (30-55d) secretion. Retinol was analyzed by high-performance liquid chromatography.

Results: Only 2% of retinol serum deficiency (<0.7 μmol/L) was found in the studied population. Milk retinol concentration was statistically different for each lactation phase (p<0.05); 2.84 ± 1.05 μmol/L in colostrum (58), 3.47 ± 1.28 μmol/L in transitional milk (58) and 2.02 ± 0.61 μmol/L in mature milk (30), reaching 97%, 118% and 69% of vitamin A nutritional requirements for preterm infants (420 μg), respectively.

Conclusions: The vitamin A nutritional status in mothers of preterm newborns was adequate according to serum retinol, with deficiency of only 2%. The vitamin A amount was higher in transitional milk, reaching the vitamin A nutritional requirements for preterm infants.
