RESUMO:

Introduction: Currently, there is no instrument available to assess the physical, social, and attitudinal environment of neonatal intensive care units (NICUs), qualifying and grading their risks or benefits. The International Classification of Functioning, Disability, and Health (ICF), when applied in targeted forms such as CORE SETS and Checklists, can be used to classify and evaluate the environmental factors involved in NICUs. Objective: To develop an instrument for identifying elements related to the health-related environmental context of patients in a NICU using an ICF-based Checklist. Methods: This qualitative study was conducted from February to December 2024 through virtual focus groups and semi-structured interviews. The study was submitted to and approved by the CEP/FACISA Ethics Committee (6.386.729). Participants included professionals from medicine, physical therapy, occupational therapy, and nursing with experience in the field, as well as caregivers of newborns (NBs) who had been hospitalized in a NICU and were up to two years old at the time of the study. Results: A total of 32 eligible volunteers were contacted for the study. Of these, 19 completed the online form, expressing their availability for the virtual group meetings. Four professionals and one caregiver were excluded due to non-attendance after three attempts to reschedule, resulting in a final sample of 14 participants (10 professionals and 4 caregivers). Data analysis generated 21 categories, covering the five chapters related to environmental factors in the ICF. Among these, 11 were classified at level two and 10 at level three, encompassing all five chapters related to environmental factors in the classification. Discourse analysis was based on identified themes, some of which were assigned to the same ICF category. However, only the theme “Effective communication between professionals and families” was assigned to two distinct categories. Conclusion: Relevant environmental factor categories from the ICF were identified in the NICU setting. The development of the checklist enables a more effective use of the classification, improving interventions, the management of NBs, and healthcare services, while also serving as a guide for creating assessment tools on this topic.
