RESUMO:

Introduction: The United Nations International Conference on Population and Development, 1994, marked the beginning of a new era for sexual and reproductive health. The Millenium Development Goals reinforced these aspects by establishing three goals related to gender equality and women’s empowerment, improvement of maternal health as well as the fight against HIV/AIDS and other neglected diseases. In most developing countries, such as Brazil, these goals are far from being met and little emphasis has been given to the aforementioned topics in the curricula of health professions. Objectives: To enhance the ethical and humanist development of students, focusing on human, sexual and reproductive rights (SRR), an undergraduate discipline was implemented, using an interdisciplinary approach and innovative methodologies. Methods: An  objective structured clinical examination (OSCE) program was developed focused on SRR to provide direct observation and feedback to students on their doctor-patient relationship skills, abilities to perform focused communication and orientation in sexual and reproductive health, and to familiarize students with this type of assessment. OSCE stations were developed to focus on case scenarios using standardized patients. The following topics were included: sexual violence, emergency contraception, infertility, abortion/miscarriage, reproductive planning, pregnancy with HIV, pregnancy with fetal malformation, postnatal care and maternal mortality. Two workshops were held with the teaching staff to select problem situations, provide training in assessment methodology and supply feedback. A total of 90 Ob/Gyn clerkship students took part in three OSCE sessions, each involving 6 different stations. Each station had duration of seven minutes and students were observed independently by two raters. Stations were designed to assess history-taking, communication, decision-making and doctor–patient relationship skills. At the end of each session, students receive appropriate feedback regarding each station. Performance assessments on 10 to 15 items per station were obtained and used to generate OSCE scores. These items were binary in that the behavior was either carried out (1 point) or not (0 points) by the student. The total number of points and percentages were calculated to determine station performance score (mean of two raters). Interrater reliability was also measured. Results: In our context, implementation of OSCE on SRR proved to be feasible for evaluating senior medical students. Examinations took on average 4 hours to apply, incurring no additional costs to the university. No significant interrater difference was observed in the scores (p>0.05). The OSCE program has been shown to be a reliable method for assessing clinical skills related to human, sexual and reproductive rights of medical students. This is the first literature report using an OSCE focused on SRR. Initial observations indicate that the use of OSCE methodology is an efficient teaching tool in this area, favoring the development of clinical skills related to sexual and reproductive health. Conclusions: OSCE has been very well accepted by students and professors alike, resulting in a positive educational impact in the assessment of clinical and communication skills required for providing quality comprehensive care in sexual and reproductive health.
