RESUMO:

Introduction: Patellar tendinopathy (PT) is a condition characterized by focal pain in the patellar tendon and dysfunction. This condition most frequently affects men who play sports involving explosive jumping activities, such as basketball and volleyball. One of the most recommended protocols for the treatment of PT is a progressive loading program focusing on quadriceps strengthening, known as Heavy Slow Resistance Training (HSR). Criticisms to the HSR protocol emphasize that this exercise program does not address known risk factors for PT. It has already been shown that athletes with PT have a stiff jump landing, lower ankle joint range of motion and lower strength of the hip extensor muscles when compared to asymptomatic athletes. It is possible that these kinetic chain deficits contribute to patellar tendon overload in athletes, and may favor the occurrence and/or perpetuation of PT. However, the effectiveness of an intervention addressing these deficits has not been investigated. Objective: To compare the effects of an intervention involving the HSR protocol with an intervention directed to kinetic chain factors in terms of pain, function and severity of symptoms, lower limb strength and flexibility and jump landing mechanics in athletes with PT. Methods: Twenty-eight male recreational athletes will be recruited for this randomized, double-blind clinical trial, divided into 2 groups: Heavy-Slow Resistance Training Group (GTLAC; n=14) and Kinetic Chain Group (GCC; n=14). Both interventions will occur three times per week for 8 weeks. Validated tools will be used to evaluate the study’s outcomes before, immediately after the intervention and 3 months after the end of the interventions. The GTLAC intervention will involve exercises with emphasis on the quadriceps muscle (Squat, Leg Press and Hack Squat), with loads that will progressively increase. The GCC intervention will involve the Squat exercise, strengthening of hip extensors and ankle plantar flexors, mobilization and stretching to improve ankle dorsiflexion and a jump-landing training, to better distribute the force dissipation in the lower limb joints. Repeated measures analyses of variance will be performed to compare the data before, after the interventions and three months after the interventions, with a significance level of 5%.
