Introduction: Syphilis is a serious public health problem, with high global prevalence, and is one of the main causes of maternal and fetal morbidity and mortality. When diagnosed during pregnancy, it should be treated during prenatal care. Marginalized and vulnerable populations, such as the black population, are the most affected. Thus, access to testing, diagnosis, and treatment is a matter of health and policy, equity, and social justice. Objective: To compare the prevalence of gestational syphilis in women according to race or ethnicity based on a systematic review. Method: This is a Systematic Review, an evaluative research with a quantitative approach, which uses the Preferred Reporting Items for Systematic Reviews and Meta-Analysis guidelines as a writing guide. The search for articles was carried out in the PubMed, Web of Science, Lilacs, Scopus, and Cinahl databases, in addition to searches in Google Scholar and Open Grey. The study protocol was registered with PROSPERO under number CRD42024605429. Cross-sectional studies that presented the prevalence of gestational syphilis according to race and/or ethnicity were included. Studies with males and children were included; studies that did not include race or ethnicity; that did not present the prevalence of access to these tests or sufficient data for calculation; or other types of studies. Information on authors, year of publication, research location, sample size, age range, mean age or predominant age range, race/ethnicity of participants, prevalence of syphilis, measures of association and data collection method were collected from the studies. The Checklist for Analytical Cross Sectional Studies Joanna Briggs was used to assess the risk of bias. All steps were performed by independent evaluators. Results: White women, when compared to non-white women, had a prevalence rate of syphilis ranging from 0.078% to 60%, while non-white women had different prevalence rates depending on skin color/ethnicity. Black women had rates ranging from 0.046% to 61.1%. Preliminary conclusions: Pregnant women had different prevalence rates of syphilis during pregnancy depending on their race, ethnicity or skin color.
