Introduction: Obesity is a chronic disease characterized by excessive accumulation of adipose tissue in the body and is listed by the World Health Organization as the 5th leading cause of death worldwide. Therefore, it is important to implement a line of care, as well as quality management in health services, to promote safety and effectiveness in the care provided to patients. One of the treatments offered within this line of care is bariatric surgery, indicated for patients with severe obesity who have not been successful with traditional weight loss methods, such as diet and exercise. Bariatric surgery has shown positive results in weight reduction and in improving comorbidities associated with obesity, such as type 2 Diabetes Mellitus and hypertension. To ensure safety and effectiveness in patient care, quality management in health services is essential, providing efficient and safe care at all stages of treatment. Objective: To evaluate and improve the quality of preoperative care management for bariatric surgery in a university hospital in Northeastern Brazil. Methodology: Quasi-experimental before-and-after study, without a control group, with a quantitative approach, to be carried out through the application of a quality improvement cycle in an university hospital in Northeastern Brazil. This study was approved by the Human Research Ethics Committee, CAAE: 83813924.5.0000.5292. To this end, the following steps were carried out: identification and prioritization of the quality problem; analysis of the causes of the problem; and definition of criteria. Partial results: The results obtained so far highlight the identification of critical factors that contribute to deficiencies in the management of preoperative care for patients undergoing bariatric surgery in a university hospital. Based on the analysis of the Ishikawa Diagram, causes related to the professional team were raised, such as the workload of the nurse in charge. Within the scope of the procedures, the difficulty of accessing imaging exams was verified, as well as the absence of standardized preoperative evaluation protocols. Furthermore, a lack of structural resources was found, such as appropriate rooms and materials for educational activities, which impact communication with patients. Difficulty in understanding instructions and high levels of anxiety due to having to visit the hospital several times were also highlighted, since the fragmentation of care causes disruptions in social and occupational life, as well as for caregivers, in addition to financial issues. These findings point to the need for specific interventions and an integrated approach to overcome the identified obstacles and improve the quality-of-care management.
