IMPROVING COMMUNICATION IN HOSPITALIZED PATIENT CARE: IMPLEMENTATION OF THE INDIVIDUALIZED THERAPEUTIC PLAN IN GERIATRICS

INTRODUCTION: A therapeutic plan is a set of therapeutic choices, using a multidisciplinary and interdisciplinary approach, developed based on the problems identified by the patient. This plan defines specific actions to solve health problems in an individualized manner, that is, centered on the person. It must contain the determination of deadlines, goals and expected results and the active participation of the client (ONA, 2022). It improves communication between the professionals involved after establishing treatment goals to be met, directing the proposed treatment more fluidly. OBJECTIVE: To develop and implement a therapeutic plan in the geriatric clinic. METHODOLOGY: This is a quasi-experimental study, of the before and after type, without a control group, through cycles of quality improvement in a multifaceted intervention. Study carried out in clinical inpatient units that receive geriatric patients in a highly complex philanthropic hospital, located in the city of Teresina-PI. The improvement cycle is being conducted through predefined steps, with the objective of assessing whether there was an improvement after the actions implemented. This cycle includes the following steps: 1) Identification and prioritization of opportunities for improvement; 2) Analysis of quality problems; 3) Definition of criteria for quality assessment; 4) Planning the study to measure the level of quality; 5) Implementation of interventions aimed at the most critical criteria; 6) Reassessment and documentation of the results of the improvements (Saturno, 2017). To assess the results, 5 previously defined criteria are being used, with data collection from patient records and 1 interview with patients. The ethical precepts of resolution No. 466/12 are being preserved. RESULTS: The results of this study are expected to consolidate adherence to the therapeutic plan in the institution and consequently improve communication, patient care and clinical outcomes, generating more quality and safety in care.
