SUMMARY:

Introduction: In Latin America and the Caribbean, cesarean delivery rates reach four in 10 (43%) births. The most common complication after cesarean section is surgical site infection (SSI), defined as an infection that occurs at the site of a surgical procedure within 30 days of surgery. The incidence of SSIs in cesarean sections varies from 3 to 15% in different countries. This complication results in prolonged hospital stays, compromising satisfactory puerperal evolution, delaying the bond between the mother/newborn binomial, in addition to increasing costs for the health system.

Objective: To carry out an improvement cycle to reduce the rate of surgical site infection after cesarean sections in the context of a university hospital. 

Methodology: This is a quasi-experimental mixed type study, with analysis, before and after, and of time series, without a control group with a quantitative approach, using a cycle of quality improvement. 8 quality criteria were elaborated, 7 of which were processes to assess adherence to good practices established and recommended by the literature, and 1 result criterion. Interventions were defined in the affinity diagram, based on the analysis of the main causes of the problem and also the result of the initial quality assessment, and the actions are monitored through the gantt diagram. The context was analyzed according to the categories of the MUSIQ model. 

Results: In the first evaluation, for the quality criteria with retrospective data collection, 112 non- conformities (NC) were verified. Considering the percentage distribution in each criterion, 100% of NC were identified for the Trichotomy performed properly criterion, 82.2% for the criterion Maintenance of sterile, clean and dry surgical wound dressing in the first 24 hours, 53.3% for the criterion Administration of antibiotic prophylaxis at the appropriate time and dose and 13.3% of non-conformities for the criterion Bath performed properly. For the criteria with observational / concurrent data collection, 42 NC were verified. Considering the percentage distribution, 100% of NC were identified for the Skin antisepsis with alcoholic chlorhexidine properly performed criterion, 87.5% for the Skin antisepsis with antisepsis chlorhexidine properly performed criterion and 75% of non-conformities for the Antisepsis criterion hand surgery performed properly. To assess the outcome quality criterion, the median SSI rate after cesarean section in 2021 was used as reference data, which corresponded to 10.9%.
