RESUMO:

Clinical deterioration in hospitalized patients often occurs and may culminate in serious events such as orotracheal intubation and cardiopulmonary arrest (CPR). However, this worsening in clinical status usually does not happen so fast. Commonly, patients who develop CPR show signs of clinical impairment 6 to 8 hours before the event. Early care by a team trained in care of critically ill patient is fundamental for improvement of morbidity and mortality. However, for early care assistance, it is necessary that the team envolved to provide care is able to identify deterioration. The heterogeneity and different degrees of qualification among professionals are certainly factors that can generate confusion in this process. To try to reduce this problem, several organizations have developed models of early warning systems, which are based on checking routine vital signs and, in case of change in values, establishing predetermined conducts through scores and rapid response teams (RRTs). These rapid response systems, so to speak, are able to detect events, generate triggers and improve response to them, involving healthcare professionals and family members, by encouraging participation in the process. The NEWS (National Early Warning Score) is one of these scores and was developed in 2013 by a multidisciplinary team of the Royal College of Physicians of London (RCPL), and has been used not only in deteriorating patients but also as a surveillance for all hospitalized patients. The objective of this project is to implement NEWS within an early warning system in the evaluation of adult patients hospitalized in cardiovascular diseases ward (cardiology and vascular surgery) of a university hospital and to measure the degree of adherence of the professionals involved, with a view to increasing safety through recognition of the patient in clinical deterioration.
