RESUMO:

The purpose of this study is to implement a Quality Improvement Program and identify the main causes related to the inadequate completion of the records of occurrences (medical records) of the Mobile Emergency Service of Patos/ Paraíba(PB). The study is divided into two stages and the process was established through the identification of the problem by the employees and a prioritization matrix was drawn up to list the main modifiable causes with a service improvement characteristic. This problem was chosen to build an improvement cycle. Two evaluations were made in the nine-month interval. In the first step, 100 randomly selected samples were selected from the random sample of 2016 and evaluated under the following seven criteria: personal identification, clinical evaluation, assessment of pathological antecedents, standardization with documented record, correct-accurate-readable record , professional responsibility for completing and Systematization of Nursing Assistance (SNA) The complete analysis of the evaluation showed that the most faulty criterion in filling was in the SNA, taking into account that approximately 90.0% of the files are filled by nurses, followed by background clinical assessment, legible letter, professional accountability, standardization and person identification. During the first quarter of 2017, interventions were carried out with corrective measures to ensure completeness. In the second phase, in 2017, a new evaluation was carried out with a further 100 fiches, which presented a new characterization, with completeness that suggests favorable quality, however, maintaining the need for new monitoring for continuity of care exercised by the team. This research is useful to implement a cycle of quality improvement in the SAMU, Patos/PB, identify failures in filling in the patient records favoring the institution data for quality improvement, as well as alert those responsible for filling in the appropriate ethical problems, and patients to provide a complete document to guarantee their right.
