RESUMO:

Background: Inflammation, and, mainly, pro-inflammatory cytokines have been pointed as key factors in the development of type 1 Diabetes mellitus (T1DM) and diabetic nephropathy. We aimed to investigate the association of polymorphisms in pro-inflammatory cytokines and toll-like receptor-2 (TLR2) genes with T1DM and markers of renal function.

Methods: IL6 (-634C>G and -174G>C), IL1B (3954C>T and -511C>T), TNFA (-308G>A) and TLR2 (1350T>C) polymorphisms were analyzed in 173 children and adolescents with T1DM and 144 normoglycemic (NG) subjects. Glycemic control (glycated hemoglobin and fasting serum glucose), lipid metabolism (serum lipids) and markers of kidney function (serum urea and creatinine, and urinary albumin-to-creatinine ratio – ACR) were evaluated.

Results: T1DM patients showed a poor glycemic control, altered lipid profile and ACR when compared to NG subjects (p<0.05). IL6 -174G>C polymorphism was associated with susceptibility to T1DM. Studying only T1DM patients, we evaluated the association of the polymorphisms with biochemical parameters in various genetic models. CC carriers for IL6 -174G>C polymorphism showed higher concentrations of ACR, total cholesterol and LDL-cholesterol, in the recessive model (p<0.05). In a dominant model, increased levels of urea and creatinine were found in CC carriers for IL1B 3954C>T polymorphism (p<0.05). For IL1B -511C>T, heterozygote patients showed higher concentrations of glycated hemoglobin when compared to homozygote patients (p<0.05). AA carriers for TNFA -308G>A polymorphism showed increased levels of HDL when compared to both GG and GA carriers (p<0.05). Finally, for TLR2 1350T>C, higher concentrations of triglycerides were found in TC compared to TT carriers (p<0.05).

Conclusions: These results are suggestive that IL6, IL1B, TNFA and TLR2 polymorphisms may contribute to T1DM and diabetic nephropathy development.
