STRATEGIES IN DIGITAL HEALTH AND THEIR IMPACTS ON THE QUALITY OF CARE IN PRIMARY HEALTH CARE IN THE CONTEXT OF COVID-19: A SCOPING REVIEW

SUMMARY:

Introduction: The COVID-19 pandemic brought social, economic and health impacts, which required rapid adaptation of health systems, not only in terms of quantity, but also in terms of quality. The use of Information and Communication Technologies tools had a portentous effect in the fulfillment of this objective, however, it is not known to what extent this technology was being incorporated by health systems.

Objective: To map the use of digital health strategies in Primary Health Care worldwide, and their impact on quality of care, in the context of the COVID-19 pandemic.

Methods: A scoping review was performed, based on the Joanna Briggs Institute manual, guided by PRISMA Extension for Scoping Reviews. A systematic and comprehensive three-step search was performed in June and July 2021 in multidisciplinary health science and gray literature databases. Two authors independently assessed eligibility and extracted data, which were analyzed using thematic analysis.

Results: The review included 44 studies. Six thematic groups were identified: Characterization and geographic distribution of studies; Nomenclatures of the digital strategies adopted; Types of Information and Communication Technologies; Characteristics of digital strategies in Primary Health Care; Impacts on quality of care; Benefits, limitations and challenges of digital strategies in Primary Health Care.

The impacts on the organizational dimension were the ones that were most present in the studies, being positive for the strengthening of the continuity of care; economic, social, geographic, time and cultural accessibility; coordination of care; access; comprehensiveness of care; optimization of query time and efficiency, mainly. On the other hand, negative impacts were observed in this same dimension, such as less access to services and evidence of increased inequities and unequal use of the services offered; digital exclusion of significant portions of the population; lack of planning in defining the role of the team of professionals; disarticulation of actions with the real needs of the population; fragile articulation between remote and face-to-face modalities; unpreparedness of professionals to meet the demands with the use of technologies.

Conclusions: The results revealed positive and negative impacts of remote strategies on the quality of care in primary care, but above all some inability to take advantage of the potential of technologies, which may demonstrate differences in the organization of rapid and emergency implementation of digital strategies in PHC around the world. Thus, given the importance of digital health in the current global health situation, and the possibility of integrating and advancing this strategy after the pandemic, primary care must strengthen its response capacity, expand the use of information and communication technologies, managing changeable challenges adaptively and anchored in evidence.
