SUMMARY:

Background: Several published studies have reported an association between participation in a food assistance program with a higher prevalence of overweight and obesity.

Objective: To compare the nutritional status and nutrient consumption among workers from companies participant and non-participant in the Workers’ Food Program (WFP) in Brazil.

Design: Observational, prospective, cross-sectional, comparative epidemiological study, based on a probability sample representative of the population of manufacturing workers in the state of Rio Grande do Norte. Random samples of manufacturing workers from companies participant and non-participant in the WFP were obtained by stratified multi-stage sampling for the assessment of body mass index (BMI) and waist circumference (WC). Nutrient consumption was assessed woth the 24-hour recall method. Statistical analysis was performed separately in each sex, using a mixed effects multilevel linear regression model, including sampling weights for companies and for workers conditional to the selection of their companies, with size as a fixed factor and with sector of activity, company nested within sector of activity and workers nested within companies as random factors.

Results: A total of 929 workers in 33 companies were evaluated (484 from non-WFP and 445 from WFP companies). In males, the WFP group had higher BMI (+1.08 kg/m², 95%CI +0.73 to +1.44, p <0.001), higher WC (+3.27cm, 95%CI +1.83 to +4.70, p<0.001) and greater prevalence of obesity (OR 1.67, 95%CI 1.31 to 2.14, p<0.001); in nutrient consumption, they had greater consumption at lunch of carbohydrates (+39.5, 95%CI +6.10 to +72.9, p=0.02), protein (+11.1 kcal, 95%CI -1.46 to +23.6, p=0.08) and fiber (+1.4g, 95%CI +1.0 to +1.8,p<0.001), and lower consumption at the remaining meals of carbohydrates (-30,5, 95%CI -40.4 to -20.7, p<0.001) and fiber (-1,2 g, 95%CI -1.8 to -0.6, p<0.001). In females, no statistical significant differences were observed in anthropometric parameters, but the WFP group had lower prevalence of obesity (OR 0.49, 95%CI 0.24 to 1.00, p=0.05), lower daily calorie consumption (-92.5kcal, 95%CI -154.3 to -30.8, p=0.003), lower carbohydrate consumption at the remaining meals (-17.5 kcal, 95%CI -30.1 to -4.9, p=0.007) and daily total (-59.3 kcal, 95%CI -117.9 to -0.8, p=0.05), lower protein intake at lunch (-14.2kcal, 95%CI -27.6 to -0.7, p=0.04) and total daily lipids (-18.6 kcal, 95%CI -36.0 to -1.3, p=0.04). Women in the WFP group also had lower sodium intake at lunch (-169 mg, 95%CI -244 to -94, p<0.001), in the remaining meals (-329 mg, 95%CI -647.5to -10.2, p=0.04) and daily total (-500 mg, 95%CI -794 to -206, p=0.001).

Conclusion: An association was found between participation in the WFP and an increase in BMI and WC among male workers; however, this association was not found in females. Regarding nutrient intake, men have a higher consumption of carbohydrates and protein at lunch, while women in the WFP group showed a qualitatively better food intake when compared to the non-WFP group. An explanation for the difference in results in both sexes may be that the beneficial effects of nutritional assistance programs on nutritional status may related to the reduction of food insecurity in low-income people rather than to the provision of food to decrease nutritional deficits.
