Association between physical activity and sedentary behavior in pregnancy with fetal weight in women with gestational diabetes: A cross-sectional study

Introduction: Gestational Diabetes (GD) is associated with increased materno-fetal morbidity and long-term complications for the mother and child. In the fetus, GD-induced hyperglicemia is associated with excessive weigth gain. Low physical activity (PA) and high sedentary behavior (SB) are associated with a higher risk for GD and large for gestational age (LGA) newborn. However, little is known about the association between PA and SB with appropriate fetus weigth in women with GD. Objective: To investigate the association between different domains of PA and SB during pregnancy and fetal weigth in women with GD. Methods: Sixty-seven women with GD (31.9 ± 5.9 years) from a prenatal care service of a reference maternity from Northeast of Brazil, between 20-35 weeks, participated of this study. Fetal weigth was estimated by obstetric ultrasound between 24-36 weeks. PA level was assessed by: i) amount of steps per day over a one-week period (pedometer-measured PA); ii) Pregnancy Physical Activity Questionnaire (duration, intensity and domains of PA and SB – household, self-care, occupational, transportation, and leisure). The participants were divided into two groups: i) women with GD and appropriate fetus weigth (n = 51); ii) women with GD and LGA fetus (n = 16). Chi-squared test and Fisher’s exact test were used for statistical analyses. Significance level was set at 5%. Results: Women with GD and appropriate fetus weigth showed higher PA level in the leisure χ2 (1) = 3.942, p = 0.047)and transportation domains (p < 0.05). Also, PA level in the leisure and transportation (χ2 (1) = 4.365, p = 0.037) domains was associated with appropriate fetal weigth. No associations were found between pedometer-measured PA and SB with appropriate fetal weigth (p > 0.05). Conclusion: The results indicate that the PA level in the leisure and transportation domains is associated with appropriate fetal weigth in women with DG, which suggests a possible protective role of PA for the mother and child.
