IMPROVING ADHERENCE TO THE CYTOPATHOLOGY TEST IN A MUNICIPALITY IN NORTHEASTERN BRAZIL

SUMMARY:

It is estimated that there will be approximately 17,010 new cases of cervical cancer in Brazil for each year of the triennium from 2023 to 2025, corresponding to an estimated risk of 15.38 cases per 100,000 women. The screening test for cervical cancer should be performed on women between 25 and 64 years of age, once a year, and after two consecutive negative annual tests, every three years. The coverage of cytopathological exams is one of the performance indicators of Primary Care; however, reaching the agreed-upon goal is still a challenge. In addition, the quality of the registry in the Electronic Citizen Record (PEC) influences the correct registration of the exam performed. The objective of this study is to improve adherence to cytopathological exams in a municipality in northeastern Brazil. Specifically, it aims to identify the main barriers to performing cytopathological exams in a municipality in northeastern Brazil; analyze the quality of exam records in the Electronic Citizen Record (PEC); and evaluate the percentage of cytopathological exams performed before and after interventions. This is an intervention research with a quasi-experimental study, of the before-and-after type, without a control case, and with a quantitative approach. The intervention will follow the improvement cycle model and will consist of the following stages: a) identification of the opportunity for improvement; b) analysis of the causes of the problem; c) quality assessment (baseline); d) planning and implementation of interventions to improve quality; and e) re-evaluation of quality (post-interventions). Absolute and relative improvements will be calculated for the indicators of cytopathological exam completion percentage and PEC completeness percentage, measured in 660 women (330 before and 330 after the intervention). The analysis of the intervention's effect will be performed using the Student's t-test, considering a significance level of 5% for one-tailed evaluations. With this study, it is expected to increase adherence to Pap smear testing, reaching at least the agreed-upon goal of 40% coverage of cytopathological exams.
