SUS EXPENDITURE ON HOSPITAL ADMISSIONS FOR CHRONIC NON-COMMUNICABLE DISEASES IN RIO GRANDE DO NORTE (2000-2024): A DECOMPOSITION ANALYSIS AND PROJECTION UNTIL 2060

In the Unified Health System (SUS), hospital admissions are a guiding element for analyzing the consumption of health services, representing a proxy for understanding the differences in the use of hospital services (inpatient beds) according to sex, age groups, diagnostic motivation/cause of hospitalization and associated expenses in total and average values. A worrying scenario of growth in total public spending on health care is emerging, highlighting the treatment of chronic diseases or conditions involving people who are more vulnerable and in need of financial resources. In Brazil, the Brazilian Northeast and the state of Rio Grande do Norte, the Demographic Transition is advancing along with an “incomplete epidemiological transition”, characterized by the triple burden of diseases. To this end, we intend to analyze expenditures on hospitalizations due to Chronic Noncommunicable Diseases (NCDs) among the adult population of the state of RN aged 30 and over, according to age and sex, in the period 2000 to 2024, projecting this scenario until the year 2060. The theoretical-methodological modeling that guides this investigation is supported by a basic tripod consisting of distinct and interconnected fields of knowledge, namely: Demography, Public Health and Health Economics. The quantitative study has as variables: (a) the number of hospitalizations in the SUS hospital network in RN of people according to sex and five-year age groups (from 30 years old to 90 years old and over), stratified by selected diagnoses of NCDs from the International Classification of Diseases - 10th Revision (ICD-10); (b) total amount in reais spent on hospitalizations performed for the target population selected above and, (c) average value of the AIH according to the same parameters of sex and age, according to the payment authorized and made based on the database of the SUS Hospital Admissions System - SIS/SUS. The decomposition using the population estimate for 2060 (IBGE, 2024) allows us to measure the impact on total expenditures under the effect of the change in the age structure in this period of time forecast, keeping the other variables constant (TU and average expenditure). The first surveys carried out for the period 2000 to 2024 point to different spending behaviors per hospitalization according to sex and also by age groups, presenting important elements of analysis for the Bed Utilization Rate and Average Expenditure on hospitalizations, stratified by the population over 30 years old. As a result, the aim is to design scenarios that broaden the understanding of “transition phenomena” and the understanding of the factors that impact the public health system. The current expectation is to promote reflections that drive changes in the modus operandi of health management, with a view to using “health, epidemiological and demographic intelligence” in the practices of management teams, guided by scientific research and support organizations in the SUS.
